Summary
This document is a standard-form Personal Questionnaire (PQ) issued by the Cayman Islands Monetary Authority, dated March 2019. It must be completed by every individual who requires CIMA approval in connection with a licence holder or licence applicant under the Banks and Trust Companies Law, Companies Management Law, Insurance Law, Money Services Law, Mutual Funds Law, or Securities Investment Business Law -- for example, proposed directors, shareholders, managers, controllers or officers of a regulated entity.
- Personal and professional information: Career history, prior regulatory approvals, professional qualifications, directorships/shareholdings, litigation, investigations, bankruptcy, disqualifications and PEP status.
- Source of wealth/funds: For certain higher-risk applicants, source of wealth/source of funds information is also required.
- Supporting documents: A CV and a certified copy of government-issued photo identification must be supplied.
By signing, the individual certifies the information is complete and correct, authorises CIMA to make further enquiries and verification checks, and undertakes -- for as long as they remain a director, shareholder, manager, officer or controller of a CIMA-authorised institution -- to notify the Authority of any material change affecting the completeness of specified answers within 21 days. Providing false or misleading information is stated to be a criminal offence that can lead to refusal or revocation of a licence.
Key obligations
- Complete the Personal Questionnaire in English with all answers typed, leaving no question unanswered (write 'Not Applicable' or 'N/A' where a question does not apply).
- Attach a curriculum vitae/resume and a notarized or similarly certified colour copy of a passport, driver's licence or other government-issued photo ID, with the certifier's signature, name, phone number, profession, business name/address or stamp, and date.
- Ensure all answers and information provided are true and correct; providing false or misleading information is a criminal offence and can lead to refusal or revocation of a licence.
- Disclose full particulars on a separate sheet for any 'Yes' answer in the Fitness and Propriety section (Section 7A).
- Complete the Declaration of Source of Wealth and Source of Funds form if required (applicable to individual shareholders/controllers holding 10% or more of issued shares, or those subject to enhanced due diligence such as PEPs or high-risk applicants).
- Certify, by signing, that the information given is complete and correct, and authorise the Authority to make enquiries and seek further information to verify it.
- Notify the Authority of any material change affecting the completeness of answers to questions 18-32 within twenty-one days, for as long as the individual remains a director, shareholder, manager, officer or controller of an authorised institution.

Deadlines
- within a period of twenty-one days: Individual must notify the Authority of any material changes affecting the completeness of answers to questions 18-32, for as long as they remain a director, shareholder, manager, officer or controller of an authorised institution.
