VIRRGIN User Declaration Form

Summary

This is a short administrative form used by the BVI Financial Services Commission (FSC) to grant an individual access to VIRRGIN, the Commission's online Virtual Integrated Registry and Regulatory General Information Network. By signing it, the named individual formally agrees to be bound by the Commission's Terms and Conditions of Use for Authorised Users.

• Purpose: Used to apply for and authorise an individual's access to the VIRRGIN system.
• Declaration: Signatory confirms they have read, understood and agree to abide by the General Terms and Conditions in the Terms and Conditions of Use (Authorised Users) agreement and any supplemental guidelines or policies between the Commission and the Subscriber employing them.
• Fields required: Print name, signature and date (DD/MM/YYYY) of the signatory.

Key obligations

• The signatory must read, understand and agree to be bound by the Terms and Conditions of Use (Authorised Users) agreement and any supplemental guidelines or policies before being granted VIRRGIN access.
