Summary
This is Part 6 of Form F100, the standard declaration page attached to an application for a Licence or Certificate as a Financial Services Business Provider submitted to the BVI Financial Services Commission (FSC). It is a signed statement by the applicant or the applicant's agent confirming the truthfulness of the application and the authenticity of supporting documents, plus an internal FSC section for recording receipt, processing and the Licensing Supervisory Committee's decision.
- Declarant: The applicant or the applicant's authorised agent signs the declaration on behalf of the applicant.
- Content of declaration: Confirms that all information in the application is true and accurate to the signer's knowledge, and that all submitted documents have been verified as authentic.
- Signer details required: Name, position, contact details (physical address, email, telephone and fax numbers) and date of signing.
- FSC internal use: Space for the FSC to record date received, who processed the application, date considered by the Licensing Supervisory Committee (LSC), the outcome (approved, denied or deferred), and reasons for denial or deferral.
The form itself imposes no ongoing regulatory obligations beyond the one time certification needed to complete a licence or certificate application; it is purely a declaratory and administrative component of the application package.
