Office Representative Form (OF-01)

Summary

This is a registration form (OF-01) used by companies to notify the BVI Financial Services Commission of an authorised office representative. The representative is the individual permitted to collect and deliver documents to and from the Commission on the company's behalf.

• Company details: The form requires the company name and the representative's personal details (name, job title, email, telephone, mobile, address).
• Temporary appointments: If the representative is being appointed temporarily, the company must state the effective From and To dates of the appointment.
• Director/Senior Officer declaration: A director or senior officer of the company must sign a declaration confirming the named person is authorised to collect and deliver documents on the company's behalf, and date the signature.
