Summary
This is Form TM2, the official complaint form used by applicants or complainants to file a non-insurance related complaint with the Financial Services Complaints Tribunal against a licensee regulated by the BVI Financial Services Commission. It is a procedural intake document, not a rule or regulation, and it is prescribed under Rules 15(2) of the Tribunal's rules.
- Purpose: Allows a complainant, or a representative acting on their behalf, to formally report a complaint against a licensee to the Tribunal
- Content required: Details of the complainant and licensee, nature and date of the claim, whether payment or settlement was offered, supporting documents attached, and any witnesses to be called
- Declaration: The signatory authorises the Tribunal to obtain information from the licensee and declares the information provided is true and complete
- Submission: Completed form, with supporting documents checklist, is returned to the Secretary of the Financial Services Complaints Tribunal in Tortola, by post, email, or phone contact for queries
