Summary
This notice from the Bermuda Monetary Authority announces a further update to the Personal Declaration (PD) Form used for proposed director and officer appointments under the Insurance Act 1978 and the Investment Funds Act 2006. It follows an earlier notice of 26 November 2025 introducing the previous version of the form.

- Cut off for old forms: From 1 February 2026, the BMA will not accept previous versions of the PD Form or any variation of it.
Entities and individuals proposing new director or officer appointments regulated under the Insurance Act 1978 or the Investment Funds Act 2006 must use the updated form going forward to avoid rejection of submissions after the cut off date.
